Tennessee is one of the 43 states or territories that have received an Aging and Disability Resource Center (ADRC) grant since 2003. The ADRC initiative is a collaborative effort of the Administration on Aging and the Centers for Medicare & Medicaid Services designed to streamline access to long term care.
Tennessee chose the name Aging and Disability Resource Connection because the public can access the ADRC through this Web based directory or by visiting, calling, e-mailing or writing to an Area Agency on Aging and Disability. The ADRC is more than a bricks and mortar “center” in Tennessee. Tennessee also chose the slogan “Choices for Independence” to show what Tennesseans want in regard to long term care.
The ADRC initiative supports state efforts to develop a “single point of entry” at the community level that will help people make informed decisions about their service and support options. The ADRC also assists individuals in navigating the long term support system.
ADRC programs provide information and assistance to individuals needing either public or private resources for themselves, caregivers, a family member, a friend, professionals seeking assistance on behalf of their clients, and those planning for their future long term care needs. An ADRC also serves as the entry point to public long term supports including those funded under Medicaid, the Older Americans Act and state funded programs.
The goal of the ADRC program is to make it easy for Tennesseans to make informed choices based on good information and to also make it easy to apply for the support they need. The vision is to have an ADRC in every community serving as highly visible and trusted places where people can turn for information on the full range of long term support options.
